"E.R.I.K."
E.R.I.K.
is an Emergency Response Information Kit developed by a
community Advisory Council, established in response to
community concerns regarding the availability of
adequate information in emergency situations. All area
emergency groups have endorsed this program. Once
completed, the kit provides the necessary information
for emergency personnel to respond quickly to your
situation.
WHO
SHOULD USE THE KIT?
The kit is ideal for seniors,
chronically ill persons, those who live alone or who
have caregivers, and others who have speech difficulties
or communication bathers. You may also want to take a
copy of E.R.I.K. and your complete Health Care Directive
with you when you are away on a trip. It is important to
update the information form as your information changes.
WHAT DOES E.R.I.K CONSIST OF?
- A
health information form, which a health care
professional, a pharmacist or your family may assist
you with.
- A
place to insert your Health Care Directive or Living
Will, if you have one. A form is enclosed if you
wish to make a Health Care Directive with your
Doctor and family.
- A
blue organ donation card, if you wish to complete
it.
- A
sticker like the one illustrated above to stick on
your front entrance.
- A
plastic folder with a magnet to hold all the
information place on the front of your fridge.
